Program Goals
- Provide eligible youth and families with timely access to a comprehensive array of home-based and evidence-based mental health services provided by credentialed individuals that are knowledgeable and competent in delivering these treatments and ensure that these treatments and interventions are provided within a system of care embodying the Hawaii Child and Adolescent Service System Program (CASSP) principles.
- Promote the use of current knowledge regarding evidence-based services in the development of individualized plans and practices to improve outcomes for youth.
- Provide eligible youth and families with home-based services delivered in their community setting to improve overall functioning of the youth and family, and to prevent the need for higher levels of treatment whenever possible.
- To re-unify the family after the youth has been placed outside the home, or to support the transition to a new DHS resource family for youth with behavioral challenges.
- To provide youth and young adults who need to work intensively on developing a range of skills to prepare for independent living.

- IILS - Intensive Independent Living Skills (helping youth and young adults develop independent living skills)

- Intensive Individual & Family Therapy aimed and developing independent living skills

- Accessing community resources including financial assistance, benefits and vocational services
- Skills training and interventions addressing social skills, emotion regulations, self care, personal finances, career planning, self-management of physical of mental health treatment
- Engaging natural supports with understanding the challenges of parenting a young person transitioning to adulthood.
- Development of a Mental Health Treatment Plan, with Crisis and Safety Plan Components

Admission Requirements
- IIH - The youth must be age three (3) through twenty (20) years;
- The youth is displaying behavioral or emotional challenges in the home/community and there is a reasonable likelihood that IIH services will lead to specific observable improvements in the youth and family’s functioning;
- IILS - The youth must be sixteen (16) through twenty (20) years;
- Youth/Young adult must be identified as needing intensive outpatient services focused on promoting growth toward independent living with in the CSP/CMP;
- Pre-admission meeting is held with the youth, family/guardian, Care Coordinator and other relevant treatment members to identify treatment targets along with expectations of family/guardian involvement in the treatment process.
- The family/guardian(s) agree to active involvement in treatment and planning meetings, and the youth is willing to participate
- Authorization by Department of Health Family Guidance Center.
- Consent of Parent or Guardian.
- Placement in program prescribed and documented as part of the child/youth’s mental health treatment plan.
- Requires participation by all providers/caregivers in planning and treatment meetings.
