Adult Group Homes - Hale Kukunaokalā
Hale Naʻau Pono provides two housing levels of care on Oʻahu. This service is funded by the State DOH Adult Mental Health Division. If you are interested in learning more about this program, please have your case manager contact us at (808) 696-4212.
Please note that referrals must be sent from your case management agency through AMHDʻs SharePoint site. If you are interested in applying for one of our homes, you must contact your AMHD or CCS Case Manager and have them submit referral forms to AMHD: DOH AMHD Universal Referral Form AND Universal Referral Form Attachment C (Housing)
Housing eligibility is determined by AMHD
Generally, Applicants must be:
- An Adult with serious mental illness where placement in this level of care is appropriate.
- MedQuest enrolled with an AMHD or Ohana CCS assigned Case Manager.
- Medical need for supportive housing (house+support services)
- Ability to pay a security deposit and monthly program fees ($385 shared or $430 single rooms)
- Ability to pay for your own food, entertainment, transportation, medical expenses.
- A desire to participate in a housing program.
Program Participants are assigned to a single or shared (limit 2 beds per room). Houses are between 4 and 7 bedrooms and 5 to 8 beds (capacity). We currently have 68 total beds across our 9 locations (Waiʻanae/Leeward side)
Program fees include room, electricity, water, internet, phone and basic cable and shared supplies like toilet paper, laundry detergent, hand soap, cleaning supplies. Homes are fully furnished and participants are expected to purchase their own food, medications, personal hygiene. Participants will work with program staff to establish personal goals, attend structured rehabilitation, education or work programs during the weekdays and perform common household chores.
Group homes are not independent living situations (rent a room or apartment on your own) or a care home (nursing/cooking/bathing/cleaning). Group Homes provide shared, safe, homelike environments where we assist adults to develop and maintain skills of independent living--managing personal finances, daily living skills, social skills, medication adherence, compliance with medical, behavioral and legal orders, and coordinating services with the various providers on the team to support client success.

8- to 16-Hr. Group Home - 2 Waiʻanae Locations
